Patient Care Coordinator RN - General Surgery Clinic
40 hours per week
Administers an assigned nursing program or organizational unit with responsibility for planning, selecting, and/or devising the methods and policies/procedures to be used and for directing nursing supervisors and/or other personnel in the accomplishment of designated goals.

Responsibilities Obtains complete nursing history on patients by means of confidential interviews with patients and family within 24 hours of admission. Performs an analysis of the patient's behavior and physical condition, and makes recommendations of care. Determines patient goals with patient, family, and other team members based on present and potential capabilities and prescribe action to achieve goals. Documents continuing care needs and coordinates all appropriate resources to ensure continuity of care upon discharge. Refers patients/families with financial or social concerns to appropriate staff members. Provides for and encourages participation by patient and family in health promotion, maintenance and restoration via patient care conferences and team rounds. Acts as a main resource for unit specialty and teaches specialty skills to team members or at Hospital based classes. Provides in-service to keep them up to date with changes or need for certification to maximize the patient's health capabilities. Performs other duties as required or assigned.
Qualifications
Required Licensure/Certifications
- Licensed Registered Nurse with NH eligibility - Basic Life Support (BLS) Certificate required.
