Pay and Benefits
The pay range for this position is $120000.00 - $180000.00/yr.

Job Description
- The Clinical Pharmacy Specialist is dedicated to the goal of contributing to the prevention and cure of cancer.
- The Clinical Specialist provides safe, evidence-based, cost-effective, and patient-centered medication management as an integral part of the Malignant Hematology interdisciplinary team.
- They are responsible for providing clinical pharmacy support to physicians, nurses, pharmacists, and other staff when caring for patients both in the hospital and ambulatory clinics.
- The Clinical Specialist utilizes a high level of expertise to recommend, design, implement, monitor, and modify pharmaceutical care plans to optimize patient outcomes.
- With extensive clinical experience within their disease state specialization, the Clinical Specialist contributes to medication safety, formulary management, pathway/guideline development, policy development/implementation, oncology reimbursement and coverage, and clinical research.
- They also play an active role in the education of patients, family members, healthcare providers, and pharmacy students and residents.
Responsibilities
- Provide oversight for safe and appropriate use of medication prescribing, dispensing, administering, and monitoring.
- Deliver clinical pharmaceutical care to optimize drug therapy for cancer patients through recommendation, implementation, monitoring, and modification of individualized treatment plans in collaboration with the healthcare team.
- Perform medication order review, clarification, and verification assessing for completeness and appropriateness.
- Supervise accurate hazardous and non-hazardous drug preparation and distribution per policies and procedures.
- Process chemotherapy orders and perform necessary double checks.
- Develop, interpret, and provide medication-related information and education for pharmacists, providers, nurses, and other healthcare personnel.
- Coordinate and provide medication therapy education and counseling to patients and caregivers.
- Precept pharmacy students, residents, and others in training.
- Participate in clinical research and projects.
- Provide leadership for developing, implementing, and maintaining medication-related guidelines, policies, and standards.
- Manage the medication use process to improve workflow, quality, and safety for patients and staff.
- Oversee the cost-effective use of medications to ensure appropriate utilization of resources and reduce financial risk.
- Doctor of Pharmacy Degree
- Board of Pharmacy Specialties (BPS) certification
- Clinical pharmacist experience
- Experience in medication prescribing, dispensing, administering, and monitoring
- Chemotherapy order processing experience
- Experience in medication therapy education and counseling
- Experience in developing medication-related guidelines, policies, and standards
- Experience with formulary management and pathway/guideline development
- Knowledge of medication safety practices
- Experience with oncology reimbursement and coverage
- PGY-2 Residency in Hematology/Oncology
- Experience in malignant hematology
- Experience with interdisciplinary teams
- Experience in clinical research
- Experience in the education of patients and family members
- Experience with policy development and implementation
- Experience with prior authorization processes
