To provide direct care to patients, under the supervision of a physician. This position is administratively supervised by the Clinical Coordinator or designee.
Essential Responsibilities:
- Assesses clinical, emotional, and educational health needs of patients.
- Assesses health needs of patients, including physical assessment.
- Collaborates with physicians plans of care and assures appropriate follow-up including telephone contact, home evaluations and hospital rounds.
- Collaborates with the physician and other clinical staff members to maintain continuity of care.
- Provides ongoing emotional support of the patient and his/her family both in formal groups and at each patient encounter.
- Participates in research activities.
- Facilitates referrals to appropriate community agencies including hospitals.
- Participates in patient education activities.
- Participates in orientation and education of nursing and support staff, and nursing and other students.
- Collaborates with physicians in developing and periodically reviewing protocols relevant to department functioning.
- Participates in planning for team functioning in delivering the total range of health services.
- Participates in monitoring quality of care according to professional standards and established criteria.
- Knows limits of practice and provides appropriate referrals.
- Carries out clinical and administrative procedures that may be promulgated by the Health Plan.
- Performs other related duties as directed.
Basic Qualifications:
Experience
- Minimum one (1) year of primary care experience is required.
Education
- Masters degree in Nursing required.
- Graduation from a NLN accredited NP program.
License, Certification, Registration
- This job requires credentials from multiple states. Credentials from the primary work state are required at hire. Additional Credentials from the secondary work state(s) are required post hire.
- Registered Nurse License (Maryland) within 6 months of hire AND Registered Nurse License (Virginia) within 6 months of hire
OR
- Compact License: Registered Nurse within 6 months of hire
- Nurse Practitioner License (Virginia) within 6 months of hire
- Certified Nurse Practitioner Certificate (District of Columbia) within 6 months of hire
- Nurse Practitioner Certificate (Maryland) within 6 months of hire
- Registered Nurse License (District of Columbia) within 6 months of hire
- Nurse Practitioner National Certificate
- National Provider Identifier
Additional Requirements:
- Proficiency in the use of applicable computer software.
Preferred Qualifications:
- Two (2) years of primary care experience is preferred.
