Salary Estimate: $142,771.20 - $214,032.00 /Year (DOE)

Job Summary:

We are seeking a Manager of Case Management to lead and support a dynamic case management team within a full-service acute care hospital in the Inland Empire. This is a leadership role focused on optimizing patient outcomes, ensuring regulatory compliance, and supporting effective hospital throughput.

The ideal candidate is a collaborative and experienced RN with a deep understanding of the case management process and a passion for delivering high-quality, patient-centered care.

Key Responsibilities:
  • Oversee the daily operations of the case management department, including staffing, scheduling, attendance, and staff support.
  • Monitor case management processes to ensure alignment with established policies, professional standards, and regulatory requirements.
  • Lead the coordination of care planning, discharge planning, and continuum of care processes.
  • Support timely patient discharges to promote optimal length of stay and reduce readmissions.
  • Foster strong relationships with healthcare providers, payers, physicians, and patient families.
  • Perform daily rounding to provide leadership and consultation to case management staff.

Qualifications:
  • Education:
    • Bachelor's Degree in Nursing (BSN) required
    • Master's Degree in Nursing (MSN) preferred
  • Licensure:
    • Active California Registered Nurse License
  • Experience:
    • Minimum 5 years of case management experience and/or nursing management in an acute hospital setting
    • In-depth knowledge of case management standards, hospital reimbursement models, and payer systems (e.g., Medicare, Medi-Cal, Managed Care)
    • Familiarity with InterQual and M&R utilization review tools
