Job Description
Job Title: Home Health Physical Therapist

Job Specialty: PT Home Health

Job Duration: 13 weeks

Shift: Day 5x8-Hour (08:00 - 16:30), Monday-Friday

Guaranteed Hours: Not specified

Experience: One year of home health experience required; DPT or Master's degree required

License: Physical Therapy license for Virginia required (can apply immediately upon offer)

Certifications: BLS required

Must-Have:
- Ability to work within a 50-mile radius
- Closed on floating to specified regions
- COVID-19 and Flu vaccines or approved exemptions

Job Description:
- Provide physical therapy services in a home health setting
- Availability to travel to specified areas 2-3 times per week with mileage reimbursement
- Train on Epic system basics for evaluations, RAs, routine visits, and discharges
- No call required
- Must work at least one holiday per season (including Thanksgiving, Christmas, New Year's)
- Orientation includes 24 non-billable hours
- Completion of orientation modules expected within specified timeframe
