Position: Head of SDOH & Behavioral Health Programs
The Head of SDOH & Behavioral Health Programs will be responsible for ownership of national SDOH and Behavioral Health program development and performance. This includes implementation and monitoring of clinical and operational protocols to support Monogram goals.
In this role, the individual will be expected to provide a high level of program and product specific subject matter expertise and consultation. This individual will be responsible for intensive oversight to ensure performance, quality, and adherence to protocols. This includes continuous evaluation of documentation systems to ensure optimized for efficiency and clinical efficacy.
The individual will work closely with Regional and Market Operations teams, and other teams across the organization.

Position Requirements
- Phoenix, AZ or Nashville, TN based, preferred or remote with travel
- Bachelor's degree from a four-year college
- Clinical background preferred, but not required
- 8+ years' experience with program management, clinical operations, value-based care, social work, SDOH, behavioral health, population health, national program execution, or similar roles
- 8+ years' experience in government programs, specifically Medicare Advantage, SNP, and Medicaid
- 5+ years' of executive leadership experience across multi-state organization
- 5+ years' of experience with product, workflow, and clinical system design
- Detail oriented, excellent prioritization, and time management skills
- Expertise partnering with internal departments to drive collaboration and results
- Highly proficient in Microsoft Excel, Word, and Powerpoint
- Experience working in high-growth environment preferred
Roles and Responsibilities
- Responsible for continuous development and oversight of our SDOH & Behavioral Health programs that defines target populations, patient interventions, and intervention effectiveness. Ability to analyze data to identify trends, ensuring our model is effectively designed and implemented to meet patient needs.
- Responsible for designing functionalities to implement patient interventions, ingesting and analyzing market feedback to optimize Social Worker user experience and intervention scalability.
- Leads Social Work Coordinator team that is responsible for executing SDOH referrals and support to Social Workers.
- Collaborates with Chief Medical Officer, Chief Clinical Officer, Vice CMO, Region Presidents, Region Medical Executives, Operations, and Finance teams to ensure cross-functional alignment on the SDOH & Behavioral Health roadmap.
- Develop and oversee programmatic requirements to meet Monogram clinical standards and outcomes.
- Responsible for oversight of policies and procedures to meet Monogram, client, and regulatory requirements.
- Continuous evaluation and improvement of systems and processes to support clinician workflows.
- Serve as subject matter expertise within the organization for SDOH, Behavioral Health, and Social Work programs and processes.
- Strong partnership with Market Operations to gain continuous feedback and successfully roll out new initiatives.
- Support Monogram as necessary with any tasks required to deliver excellent personalized polychronic care and perform all other duties as assigned.
