Job Description
Job Title: Hospice Registered Nurse (RN)

Job Specialty: Hospice Care

Job Duration: 12 Months

Shift: Day - Monday through Friday, 5x8 Hr

Guaranteed Hours: 40 Per Week

Experience: Minimum of one year as an RN in acute care, oncology, long-term care, hospice, or home health. School internships and clinical hours may be considered as experience in individual cases.

License: Current license to practice nursing in the requested state of employment.

Certifications: BLS (American Heart Association), Current automobile insurance, and valid driver's license.

Must-Have: Transportation to be used for work.

Job Description:

- Provide direct patient care services to hospice patients and supportive care to the patient's family.
- Manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff.
- Assure highest quality of care is provided and documented in accordance with the Plan of Care.
- Perform initial and ongoing assessment of the patient.
- Help develop and implement the Hospice Plan of Care in conjunction with other members of the Interdisciplinary Group.
- Work closely with the Interdisciplinary Group to provide inclusive physical, psychosocial, educational, emotional, and spiritual services.
- Report changes in patient condition to both the Attending Physician and Hospice Physician.
- Maintain an accurate and updated medical record.
- Practice infection control measures and standard precautions.
- Monitor scheduling of all staff.
- Participate in Quality Assessment and Performance Improvement Program.
- May be requested to participate in agency on-call schedule.
- Consistently promote company's core values.
- Complete required compliance annual training.
- Attend staff meetings and perform other assigned activities.
- Maintain knowledge regarding management of pain and symptoms in the dying process.
- Make routine supervisory visits of, and with, all other team members at least monthly.
- Perform ongoing review and follow-up on assigned patient clinical records.
- Maintain close contact with the patient's family and/or significant others.
- Maintain primary control and professional management of each patient.
- Provide education to the patient, family, or other caregivers in relation to identified needs on the Plan of Care.
- Supervise licensed practical nurses and hospice aides providing delegated nursing services.
