Job Description
Job Title: Physical Therapist Assistant - Home Health
Job Specialty: Therapy/Rehabilitation
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: None
Experience: Minimum 1 full year of Adult Home Health experience required
License: License must be in hand unless it takes only a few days to obtain
Certifications: CPR, BLS
Must-Have:
- Reliable transportation
- Current driver's license
- Agency-required liability insurance
Job Description:
- Conduct home visits and periodic office meetings
- Average of 5-6 patients per day
- Points-based system with 30 points required per week
- Each type of visit and activity holds a different point value
- Responsible for documentation and travel
- Assigned between 5-10 patients per day, equating to roughly 6-8 points
- Types of cases include Start of Care and Follow-Ups
- Must be willing to travel up to 75 miles from the home clinic
- Mileage reimbursed at the IRS Standard rate (compensation between patient homes; commute to and from first and last patient not billable)
- Expected to cover Plymouth territory with focuses on Southern towns including Plymouth, Wareham, Carver, Kingston, and Pembroke
- Expected weekly travel of approximately 200 miles
- Productivity expectations are set at 30 weekly units, with 7 visits per day
- Use of HomeCare HomeBase EMR system
- Open to local and traveler candidates at the same rate
- Must have recent Home Health experience, as first-timers are not accepted
- Weekend requirements may vary; discuss during the interview
- On-call requirements vary per job/location; discuss with the manager during the interview
