Job Title: Advanced Practice Provider
Job Specialty: Family Nurse Practitioner, Physician Assistant (Family Medicine)
Job Duration: 33 weeks
Shift: Day - 5x8 - 09:00AM - 05:00PM
Guaranteed Hours: 40 hours per week
Experience: 3 years in a similar setting
License:
- Physician Assistants: CA Controlled Substance Education Course Certificate
- Nurse Practitioners: Furnishing License with Schedules II
Certifications:
- Active DEA required before start
- BLS required
Must-Have:
- Month & date on work experience in CV with all employment gaps accounted for
- Credentialing immunizations (Flu Vaccine, TB Status/X-Ray, MMR Titer or Series, Hep B Titer or Series, Varicella Zoster Titer or Series, Tdap)
Job Description:
- Involvement in Chronic Pain Management
- Ensure chart closure within 72 hours
- Schedule includes seeing a minimum of 18 patients per day
- Work Monday to Friday, 8:00 AM to 5:00 PM
- Patient demographic mix: 70% pediatric, 30% adult
- Decision on applications typically made within 7-14 days
- Credentialing process takes approximately 45-90 days
- Facility requires providers to be board certified
- Must provide a 30-day notice for ending an assignment
