MAJOR RESPONSIBILITIES
- Assessment and referral coordination with the healthcare setting and community partners.
- Drives regional-wide increases in screening for social risk factors, referrals to community-based organizations and closed loop follow-up, including knowledge of Salesforce.
- Develop and maintain an updated directory of community resources and services.
- Coordinate and manage closed-loop referrals to community resources, ensuring follow-up and service utilization to improvement in patient outcomes in target populations.
- Establish and maintain collaborative relationships with community organizations, social service agencies, and other relevant stakeholders.
- Facilitate partnerships aimed at addressing racial disparities and closing clinical care gaps.
- Advocate for patients' access to community resources and support services.
- Analyze data related to social needs, service utilization, and health outcomes to identify trends and inform program improvements
- Provide ongoing support to patients to ensure they can access and benefit from community resources.
- Educate healthcare providers on the importance of addressing social determinants of health and the available community resources.
- Provide training and support to healthcare staff on referral processes and the utilization of community resources.

- Establish referral communication strategies for community partners.
- Assist with internal and community-based organization outreach and relationship building.
- Provide assistance with training on the Find Help platform.
- Assist with organizing Find Help community organization data.
- Communicate with Community Health Worker team members to facilitate client care.
- Identify community resources for clients to overcome barriers to care and facilitate access to community resources, including locating housing, food, clothing, prenatal classes, parenting, and providers to teach life skills, and relevant mental health services.
- Remain aware of current services offered by service providers, such as mental health, housing, and employment assistance.
- Works to reduce cultural and socio-economic barriers between community members and institutions.
PHYSICAL REQUIREMENTS AND WORKING CONDITIONS
- This position may require travel, therefore, will be exposed to weather and road conditions.
- Operates all equipment necessary to perform the job.
