The Virtual Care Operations Team delivers and supports an enterprise approach to virtual care best practice, ensuring a seamless delivery of quality virtual care for patients, providers and the community. Its focus is to support virtual care as an integrated part of the clinical delivery model, where the patient and provider partner to reimagine the care experience. The Virtual Care team also includes the Virtual HomeGIM Clinic and other virtual care initiatives, with the intention to support patients and their families across HHS.

The Virtual Care Manager role will report to the Urgent and Ambulatory Care Portfolio. This position includes corporate and portfolio accountabilities, supporting both inpatient and outpatient areas related Virtual Care.

Position Summary

The Virtual Care Manager will serve as a leader, coach and project manager for the implementation, advancement, integration and evaluation of virtual care across HHS. Reporting to the Director, Urgent and Ambulatory Care, the Manager will advance and strengthen virtual care, shifting virtual from a pandemic response to integrating virtual care practices into HHS clinical service delivery models. This role is highly integrated with the Ambulatory and Community Systems Task Force and Models of Care Task Force under the Workforce Planning Strategic Plan. As a co-lead of the Digital/Virtual Health working group, the Virtual Care Manager continues to play a key role informing Epic optimization. The Manager will provide oversight and coordination of the Virtual Care Operations Team members and their associated accountabilities such as managing the virtual project intake process, supporting teams in their transitions to virtual care and development of virtual care policies and standard work; execute the long term Virtual Care strategy at HHS; and lead and promote the evaluation of virtual care offerings across the organization.

The Manager will develop and foster key relationships across HHS and externally to advance and sustain the virtual care strategy at HHS. This will include but is not limited to strengthening partnerships with corporate support services, clinical and physician leadership, and ensuring patients and families are integrated into the development and design of virtual care models.

The Manager develops and leads virtual care innovative programs including the Virtual HomeGIM Clinic and Virtual Nurse-to-Nurse Support Line. This includes program strategy and development, leadership of staff, and program evaluation for success.

In alignment with the HHS remote work policy, the potential for a hybrid work model is negotiable dependent on patient and clinical need.

Qualifications

1. Undergraduate Degree-Health Related Discipline, Busine

2. 5+years of management experience in health care sector, preferred

3. 5+ years' experience leading large, complex initiatives from multiple perspective

4. Experience in leading teams to successfully achieve outcomes of a complex nature

5. Knowledge of Provincial Health care initiatives and government strategies related to virtual care an asset

6. Demonstrated excellence in leadership, organizational, team building, critical thinking and conflict resolution skill

7. Demonstrated knowledge and experience with providing and/or leading virtual care services within a healthcare environment, preferred

8. Demonstrated skill experience in Quality Improvement principle

9. Knowledge of the roles of the interdisciplinary health care team and how they interact

10. Demonstrated experience in and knowledge of Project Management principles and their application

11. Demonstrated creativity and innovation in the delivery of patient care service

12. Demonstrated excellence in the ability to initiate, model and support change in a constructive manner

13. Ability to advocate, motivate and influence others in order to gain their support and commitment

14. Demonstrated ability to collaborate and engage constructively with internal and external stakeholders at all levels of an organization

15. Skills of a "Self-starter" who can work with limited guidance

16. Excellent Verbal and written communication skill

17. Excellent presentation skills and use of power point

18. Able to deliver services both onsite or virtually
