Responsibilities

Job Summary
The System Manager Continuum of Care Virtual Nursing manages and leads the Care Base virtual RNs and telehealth nurses. This position provides clinical management to support virtual programs and staff to provide and coordinate care to individuals and families using a holistic approach. This position is responsible for leading the telehealth and virtual RNs team in collaborating with healthcare team members and providing an overall understanding of the continuum of care needs, safety, quality and patient experience outcomes. This position is responsible for effectively leading and
managing staff to facilitate progression of care, including education, assessment of clinical status and provision of patient interventions. This role directly supports the organization's efforts to enhance the patient experience, promote patient safety and improve risk reductions.

The System Manager, will provide support and oversight to virtual RNs and telehealth nurse activities and responsibilities ensuring provision of care across the care continuum. This role is responsible for ensuring regulatory compliance to federal, state and county rules and regulations; compliance to Accreditation body standards and CommonSpirit Health policies and procedures. This position will work closely and in collaboration with staff, leaders, and executive management to ensure a seamless and high performing approach to the patients they serve. A high degree of
professional competence and autonomy will be demonstrated in performing the roles 1) clinician, 2) educator, 3)consultant/collaborator, 4) leader.

Essential Key Job Responsibilites
  • Compiles data for the purpose of quality improvement and compliance with organizational and department policies, work aids, and algorithms
  • Holds nursing and other staff accountable to the established standards
  • Assists in creating and managing team schedules
  • Lead and participate in the development and implementation of new programs and initiatives
  • Supports the System Director in preparation of reports, program material, research, data collecting and analysis for various meetings and/or programs

Job Description
  • Actively participates in Care Base program growth and improvement focusing on virtual RNs and telehealth nursing
  • Manages virtual and telehealth RNs in accordance with the established work aids and algorithms and support staff in learning and upholding these work processes
  • Provides supervisory and training support to nursing, vHC and other staff
  • Creates an "expectation of excellence" and manages performance of direct reports to maximize the value of employees' contributions towards organizational success.
  • Establishes strategically aligned goals with staff.
  • Encourages and creates opportunities for continuous improvement of staff by providing frequent coaching, mentoring, and accurate performance feedback.
  • Provides direction and support to staff to deliver quality services with a focus on organizational success.
  • Ensures appropriate staffing levels are maintained.
  • Recruits and retains qualified, productive and customer-focused employees.
  • Interviews, hires and orients direct reports.
  • Monitors recruitment and retention activities and costs.
  • Provides fair and equitable treatment of all employees.
  • Creates a high performing work environment characterized by transparent and open communication, mutual trust and respect.
  • Provides education and training for employees in support of career goals and the organization's mission and strategies.
  • Maintains positive staff morale, observable through team spirit, a sense of belonging and recognition of accomplishments.


Qualifications

Required Education and Experience
  • RN required, Bachelor's in Nursing degree required.Master degree, preferred
  • Minimum of 10 years of recent experience as licensed nurse
  • Minimum 5 years of management/leadership experience
  • Minimum 5 years clinical home care nursing experience in both MCR/MCD

Required Licensure and Certifications
  • Current license Registered Nurse
  • Nurse Licensure Compact (NLC) or multi-state licensure


Required Minimum Knowledge, Skills, Abilities and Training
  • Knowledge and ability to facilitate patient care across the continuum
  • Effective communication skills
  • Excellent and ongoing knowledge of Federal and State regulations and ACHC standards.
  • Experience in establishing and maintaining behavior and performance expectations.
  • Knowledge and use of multiple EMRs
  • Ability to use multiple technologies; audio/visual
  • Ability to function and lead independently with self initiative and discipline.
  • Excellent organizational, time management and administrative skills, including the ability to set priorities, manage multiple demands effectively, use good judgment, as well as work independently and interdependently to achieve positive and timely results.
  • Demonstrate ability to effectively lead, train, develop, evaluate and manage employees
