The UW MEDICAL CENTER - MONTLAKE PHARMACY DEPARTMENT is seeking a highly-motivated CLINICAL PHARMACIST - GENERAL MEDICINE.

WORK SCHEDULE
Full-Time / 40 Hours Per Week
Rotating/Variable Shift

POSITION HIGHLIGHTS
Our clinical pharmacists provide extensive pharmaceutical care, and are a vital part of the inpatient multidisciplinary patient care team
In addition to direct patient care responsibilities, our pharmacists provide drug therapy consults, participate in treatment protocol development, provide patient/family/staff education, facilitate transitions of care, precept pharmacy students and residents, and perform duties of a Pharmacist 2

PRIMARY JOB RESPONSIBILITIES
Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies.
Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed.
Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations.
Provide drug information and education to medical teams, nursing, and dietitian staff.
Provide patient/family counseling and education. Update patient medication lists and reconcile medication orders at each visit.
Provide primary preceptorship of pharmacy residents and student pharmacists.
Perform duties of a Pharmacist 2.

Strong candidates will have completed an ASHP-accredited PGY2 residency in oncology pharmacy practice.

REQUIRED POSITION QUALIFICATIONS

Two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience.

LEGAL REQUIREMENT

State of Washington Pharmacist License.
