POSITION SUMMARY
Under the supervision of the Program Supervisor, the Mobile Residential Support Team's Licensed Clinician is responsible for assisting in the development of Support Plans and Relapse Prevention Plans. This qualified, specially trained team member will be available to provide crisis intervention, direct support and any other assistance that these individuals need while participating in MRST services. The Licensed Clinician will provide individualized supportive counseling and conduct assessments as needed or required. The Licensed Clinician will conduct quarterly workshops for the individuals intended to provide education and deepening their understanding of mental health diagnosis and related topics. The Licensed Clinician shall also be asked to undertake projects aimed at expanding and improving the scope and quality of services provided by the Mobile Residential Support Team.
DUTIES, RESPONSIBILITIES, AND ESSENTIAL FUNCTIONS
Completes all requisite documentation to support the services provided by the MRST and implements all plans developed.
Completes progress notes and Support Plans accurately, thoroughly, and in a timely manner.
Completes assessments when required.
Provides crisis intervention, direct support and any other assistance that the individuals need while participating in site-based services.
Responsible for working with the individuals on their person-centered recovery and safety plans which will align with the residential treatment plan developed in the affiliated Supportive Housing program and Home Health Coordination plan, if applicable.
Responsible for providing individualized supportive counseling to the program participants.
Participates in the review of referral information when applicable.
Participates in weekly case reviews with the other members of the MRST.
Advocates for the needs of the individuals in team discussions, communications with collateral organizations, and other contacts
Relays individual and/or program specific information to appropriate agency personnel
Ensures that all required documentation (paper and electronic) is completed /processed appropriately, in a timely manner, and meets quality and compliance thresholds.
Performs other duties as assigned.

QUALIFICATIONS
Education:
o Master's Degree in Social Work or Mental Health Counseling required
Experience:
o One (1) year of relevant experience required.
o Relevant experience can include, but is not limited to, providing support to and engagement with persons with mental health issues, completing assessments, and working with outside treatment providers and mental health professionals.
o Unpaid or paid clinical internships can be included to meet the one-year experience minimum requirement.
Specialized Training, Licenses, Certifications:
o LCSW or LMHC required
Knowledge, Skills, Competencies, and Abilities:
o Demonstrates excellent organizational and communication skills
o Critical thinking and sound judgment skills are necessary
Other Requirements:
o Valid Driver's license and ability to be an agency driver
o Submit to Fingerprint/Criminal background check
o Adheres to HIPAA Policies & Procedures/Compliance Plan
o Required to take and maintain compliance with all mandatory trainings
WORK ENVIRONMENT
This role is performed in-person. Access to Bethpage location to complete documentation. 80% of work day will be travelling and completing site-based therapeutic services at scattered site Supportive Housing apartments.
PHYSICAL REQUIREMENTS
Employee is required to sit, walk and stand for prolonged period of time (at least 8 hours).
Employee is required to effectively communicate both verbally and in writing.
Employee is required to operate standard office equipment such as computers, phones, photocopiers, fax and filing cabinets.
Employee is required to reach with hands and arms.
Employee is required to bend, stoop, kneel, crouch or stand on a stool as needed.
Employee is required to be able to lift 50 pounds at a time.
