JOB RESPONSIBILITIES
- Complete supervisory visits and starts of care admissions.
- Must be willing to travel with in the Treasure Coast area
- Provides safe and effective personal care for the client based on individualized Plan of Care prepared by nurse and available in the clients home.
- Drives/escorts client to and from appointments/activities as requested, resulting in a safe and timely transport
- Provides prompt communication of changes in client medical status/emergencies to Nurse Supervisor resulting in effective management of cases.
- Provides safe and effective nursing and personal care for the client, based on the nursing scope of practice and individualized Plan of Care in conjunction with Nurse Supervisor/Manager of Clinical Practice. May include, but not limited to, medication administration, transferring & positioning client, catheter care, effective communication with the client, family, and Nursing Supervisor.
- Drives/escorts clients to and from appointments/activities as requested, resulting in a safe and timely transport.
- Occasionally completes client assessments and supervisory visits.
- Demonstrate empathy, caring and self-confidence; ability to pay attention to details resulting in thorough follow through; anticipate problems in advance and know whether to diffuse or escalate; prioritize and demonstrate a sense of urgency.
REQUIRED EXPERIENCE & EDUCATION
- Current RN or LPN license
- Current Level 2 on the AHCA Clearinghouse
- Current TB (within last 12 months) or Chest X Ray (within last 5 years)
- Home Care Experience
- Excellent communication skills; team player; interpersonal skills; good collaboration skills; customer service skills; ability to use the electronic health record for documentation and communication regarding client care.
