We are seeking a motivated and dynamic Patient Care Advocate to join our sales team and help us expand our reach and impact in the community.
Job Summary:
As a Patient Care Advocate in our sales team, you will play a crucial role in promoting our home health services and securing contracts to grow our territories. You will be responsible for maintaining a pre-designated number of admissions each month, collaborating with our internal operations team, and ensuring high levels of satisfaction among our referral partners. This position requires a self-directed individual who can manage an average of 50 accounts, troubleshoot issues, and coordinate care effectively.
Key Responsibilities:
- **Sales and Promotion: ** Actively promote our home health services to potential clients, referral partners, and healthcare professionals to secure contracts and grow territories.
- **Admissions Management: ** Maintain a pre-designated number of patient admissions per month, tracking progress and adjusting strategies as needed to meet goals.
- **Collaboration: ** Work closely with the internal operations team to ensure smooth admissions and high-quality patient care, addressing any operational challenges that may arise.
- **Account Management: ** Independently manage an average of 50 accounts, monitoring admissions workflow and ensuring referral partners are satisfied with our services.
- **Communication: ** Regularly communicate with families, case managers, and other healthcare workers to coordinate care for patients and address any concerns or issues.
- **Field Visits: ** Conduct 5 to 7 in-person visits to accounts daily, building relationships and fostering trust with referral partners and clients.
- **Issue Resolution: ** Troubleshoot and resolve any issues that arise between our team and referral partners, ensuring a positive experience for all parties involved.
- **Documentation: ** Maintain accurate records of interactions, admissions, and follow-ups.
**Qualifications: **
- Bachelor's degree in healthcare, business, or a related field preferred.
- Previous experience in sales, healthcare, or patient advocacy is highly desirable.
- Strong interpersonal and communication skills, with the ability to build rapport with diverse stakeholders.
- Self-motivated and able to work independently while managing multiple accounts and priorities.
- Excellent organizational skills and attention to detail.
- Work well with team members
- Proficient in Microsoft Office, Careport, EMR systems, Medical Terminology preferred
Salary: $90K + Based on Experience
