Job Summary: POCAAN (People of Color Against AIDS Network) is a non-profit organization founded in 1987. POCAAN has been centered around HIV prevention and treatment, removing stigma related to HIV and reducing barriers to care, and increasing access to healthcare and improving health equity in communities of color. The Registered Nurse (RN) will provide high-quality patient care by assessing, planning, implementing, and evaluating individualized client care plans. The RN will work with and collaborate with Nurse Practitioners, and other healthcare professionals, and support staff to ensure optimal client outcomes.

Salary: 94K - 97K ( $45.20/Hr - $46.70/hr)

Key Responsibilities:
  • Patient Care:
    • Perform client assessments, including taking medical, social, and medication histories and performing physical examinations.
    • Administer treatments and foot care per protocol and standing orders.
    • Monitor and document client progress and responses to treatments in POCAAN's EMR.
    • Provide education and support to patients and their families regarding health conditions and care plans, and staff education.
  • Collaboration:
    • Collaborate with healthcare team members to develop and implement patient care plans around senior care, foot care and, when needed, phlebotomy.
    • Communicate effectively with patients, families, and other healthcare professionals.
    • Participate in interdisciplinary team meetings and contribute to patient care discussions.
  • Documentation:
    • Maintain accurate and up-to-date patient records in compliance with legal, ethical, and institutional standards.
    • Document all patient interactions, treatments, and changes in patient condition in the EMR.


    Qualifications:
    • Education: Bachelor's degree in nursing (BSN) preferred; associate's degree in nursing (ADN) or diploma from an accredited nursing program accepted.
    • Licensure: Current state RN license required. [Additional certifications may be required based on the specialty, e.g., BLS, ACLS].
    • Experience: 5-10 years of clinical experience preferred. Experience in foot care, wound care, and senior care is a plus.
    • Skills:
    • Strong clinical and critical thinking skills.
    • Excellent communication and interpersonal skills.
    • Ability to work effectively both independently and as part of a team.
    • Proficiency in using electronic medical records (EMR) systems.

    Work Environment:
    • May include working in various healthcare settings such as clinics, nursing homes, and
    • outdoor events.
    • Exposure to potentially hazardous conditions, including bodily fluids and infectious materials, with appropriate protective measures in place.
