Description
RESPONSIBILITIES:
Under the supervision of The Director of Operations and Clinical Services or the North Bay Clinical Director, the Clinical Care Coordinator provides support and assistance to clients in the Napa Supported Living Program (SLP). Generally, the Clinical Care Coordinator has primary responsibility for advocating for and responding to the treatment and rehabilitation needs of the SLP program clients. Specifically, the Clinical Care Coordinator perform the following functions:
I. Essential Functions
Direct provision of psychiatric services; assessment and evaluation of client's mental health needs and conditions Formulating and documenting assessments, diagnoses, problem lists, and progress notes into the clinical record Establish and maintain effective relations with clients, client families and support people, and with the general community; and maintaining comfortable, safe, non-institutional interventions. Conduct home visits to client dwellings, apartments, shared housing arrangements, homeless shelters and other unsheltered locations. Provide direct psychiatric assessment of all newly admitted clients, the on-going monitoring and care of all clients being served by the SLP program. Provide service in a manner that is safe and helpful for clients, staff and the general community, and that evidences a non-institutional character that is congruent with Social Rehabilitation (SR) values and principles. Develop and maintain effective relationships with personnel from Napa County Behavioral Health, Community Partners, and providers of basic supports such as housing, temporary shelter, healthcare, and financial resources. Provide crisis interventions including responding to psychiatric emergencies when required in a professional, thorough, and timely manner.
Engaging clients with developing and achieving goals with maximum client participation. Initiate and participate in discharge planning, so that clients can transition smoothly to the next level of care or the community Relay clinical information to other staff at staff meetings, and at other appropriate times. Attend and participate in staff meetings, supervision sessions, and case consultation meetings, as scheduled. Consult with the Director of Operations and Clinical Services and the North Bay Clinical Director, as necessary, on such issues as case review, medications, group dynamics, etc. Support clients with maintaining individual residences. This includes modeling investigating problems and performing minor cleaning, repair work, and other household tasks, and reporting more serious maintenance problems in a timely manner to appropriate entities. Report any suspected abuse of clients directly to the appropriate agencies as outlined in policy and law and per the adult abuse reporting document signed upon hiring. Promote the social rehabilitation philosophy among program staff and clients. Comply with all applicable federal, state and local governmental laws and regulations, as well as all agency policies and procedures, including the code of ethics. Perform other related duties as assigned by the Director of Operations and Clinical Services. Inform the Director of Operations and Clinical Services or North Bay Clinical Director of any personal events and/or circumstances which may affect the ability to perform the proscribed job responsibilities and functions or to meet minimum qualifications.

Qualifications
REQUIRED QUALIFICATIONS:
- Master's Degree in Social Work or Counseling Psychology and current registration as ACSW, AMFT, or APCC with the CA Board of Behavioral Sciences (BBS) (or registered as an associate with the BBS within 60 days of hire) or current LCSW, LMFT, or LPCC license with the BBS
- Perform behavioral health assessments, diagnoses, and interventions independently.
- Communicate effectively with medical and mental health staff, and other service providers both orally and in writing.
- Have the willingness and ability to actively, enthusiastically and effectively engage psychologically distressed clients and maintain a respectful and responsive attitude while providing therapeutically sound services. This includes being respectfully responsive to persons of differing ethnic, cultural, gender, socioeconomic, and sexual-orientation backgrounds.
- Demonstrated on-going emotional maturity, good judgment and effective teamwork.
- Demonstrate ongoing clearance with the California State Department of Social Services, Community Care Licensing Division, allowing direct contact with program clients.
- Maintain training certification and demonstrated capacity to perform basic First Aid and CPR.
- Able to work proficiently on computers and computer-based information systems, including basic knowledge of Work, Excel, and Outlook.
