Job Title: PMHNP / FNP - Nurse Practitioner (Behavioral and Family)
Location: Glendale, AZ (Hybrid) (Although this role is Hybrid, you must live close to the Glendale area since you will need to visit clinic periodically)
Hours & Schedule: Full-Time (3-4 Days/Week Schedule with Full-Time Pay & Benefits)
Work Environment: Hybrid Setting - Combination of In-Clinic and Telehealth Services
Salary / Hourly Rate: $120,000 - $150,000 / year

Job Summary:

The PMHNP / FNP (Psychiatric Mental Health Nurse Practitioner / Family Nurse Practitioner) serves in a dual capacity, offering comprehensive care that blends mental health treatment with family-focused medical services. Working with teens and adolescents facing challenges such as depression, anxiety, PTSD, and substance use, this role is both clinically rich and emotionally meaningful. The practitioner will operate as part of a multidisciplinary care team to ensure safe, evidence-based, and impactful care.

Job Duties & Responsibilities:
  • Perform comprehensive assessments and make diagnostic decisions, taking into account both mental health and physical health status
  • Provide integrated care including preventive services, chronic condition management, acute care, and health education to adolescent patients
  • Prescribe and manage medications with consideration to each patient's clinical history and behavioral needs
  • Collaborate with therapists, physicians, and support staff to build and implement individualized care plans
  • Offer psychoeducation and counseling to patients and families navigating mental health and substance use concerns
  • Coordinate care with community partners and other external providers as appropriate
  • Maintain accurate and timely clinical documentation in electronic health records
  • Participate in team meetings, quality improvement initiatives, and integrated care program development
  • Uphold compliance with regulatory standards, confidentiality laws, and ethical guidelines
  • Engage in ongoing professional development to stay informed of advancements in psychiatry and family care
Prerequisites / License & Certification Requirements:
  • Dually licensed as a Family Nurse Practitioner (FNP) and Psychiatric Mental Health Nurse Practitioner (PMHNP)
  • Active Arizona State License required

  • Located in Arizona Currently (ideally West Valley)
