Job Description
Job Title: Registered Nurse
Job Specialty: Hospice
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: None
Experience: Minimum of 1 full year in Adult Home Health Experience or Hospice experience
License: Must have a valid nursing license
Certifications: CPR, BLS
Must-Have: Proficiency in HomeCare HomeBase and OASIS, reliable transportation, valid driver's license, and auto insurance
Job Description:
- Provide, coordinate, and direct the provision of home nursing care according to physicians' orders, based on agency policies and procedures through the application of the nursing process.
- Responsible to the Clinical Manager, directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and attend periodic office meetings.
- Travel up to 75 miles from the home clinic, and must be willing to use personal transportation with mileage reimbursed at the IRS standard rate.
- Handle an average caseload of 5-8 patients per day, with documentation and travel activities contributing to productivity points.
- Types of cases include Start of Care, Wound Care, Follow-Up Visits, etc.
- Must complete all documentation within 24 hours of the appointment with the patient.
- Participate in on-call duties occasionally, 1-2 times a month.
- Must be open to traveling to another nearby clinic if weekly patient counts are insufficient to meet financial needs.
- Be comfortable with longer driving distances (50-75 miles radius may result in drives longer than 20 minutes between patients).
- All professionals must have experience using HomeCare HomeBase and have utilized it within the last 6 months, or be willing to complete a retraining course.
- Must be open to on-call rotations and accepting patients regardless of their location concerning the previous appointment.
- Complete orientation with 0 non-billable hours.
Note: Local candidates are accepted and must possess the required experience and certifications.
