The role you'll contribute:
The Director of Population Health and Care Navigation position serves as a leader and collaborator with responsibility for developing and managing Shawnee Mission Health's consumer care navigation model as well as all current and future population health initiatives, which include but are not limited to Comprehensive Primary Care Plus (CPC+), Centrus Health Clinically Integrated Network, and other physician network population health strategies. The position provides strategic oversight and execution of all physician network population health and care navigation processes that achieves brand loyalty, business growth, competitive advantage, market relevance and sustainability through the development and expansion of products and services across an Integrated Delivery Network (IDN). Position is responsible for increasing consumer access, loyalty and preference for the Shawnee Mission Hospital and Physician network of care while increasing acute and non-acute volumes and market share in the Mid-America region. The position works collaboratively with Shawnee Mission leadership and their teams for Acute Care, Non-Acute Care, Finance, Physician Network, and Patient Outcomes to achieve market performance. Position also works collaboratively with Division hospital CEO's and C-Suite Executives, business development teams, marketing teams, operational teams, as well as AHS corporate team members. Position reports to the Shawnee Mission Health Physician Network Vice President.

Strategically develops, manages, and refines the Mid-America Consumer Navigation model to create and achieve business growth, competitive advantage, market relevance and sustainability through the development and expansion of products and services across the Mid-America Division Integrated Delivery Network
Works in a matrixed and collaborative decision-making approach with stakeholders in Acute Care and Non-Acute Care Service organizational structures to assess organizational needs and develop and implement a care navigation model for the organization.
Collaborates with Mid-America Executive Leadership and teams to ensure a clear understanding of consumer care navigation recommendations and the implications of alternative solutions and financial impact to enable informed decisions.
Develops and implements an effective Consumer navigation infrastructure across the Mid-America Division which will accomplish the goal of "never discharging a patient" across the healthcare continuum. This includes monthly tracking and reporting metrics for care navigation that support the strategic organizational goals.
Develops and manages of effective tracking mechanisms and metrics-based outcomes to determine viability and performance of the Population Health revenue stream.
Prepares and manages operational budgets based upon corporate guidelines and departmental budgets prepared by departmental managers.
Qualifications
The expertise and experiences you'll need to succeed:
Minimum qualifications :
Master's of Science in Nursing
Master's in Business Administration or Health Care Administration
5+ years of experience in hospital operations, business development, physician strategy, general management is highly
Must have experience managing a team for result
Nurse Practitioner with PCP clinical experience
Licensed Nurse Practitioner
Preferred qualifications:
Working knowledge of consumer strategies in a medical environment
RN - Registered Nurse - State Licensure and/or Compact State Licensure or will accept proof of temporary RN license
