Dignity Health's Pacific Central Coast Health Centers (PHC) is a non-profit Community clinic organization comprised of more than 40 health centers located primarily from Ventura to Templeton spanning the coast. Responsibilities
In keeping with the Pacific Central Coast Health Centers health care philosophy, the Care Team Access APP is credentialed with advanced training in areas of primary health care for adult and pediatric patients, including, but not limited to: taking health history, performing physical examination and psychosocial assessments, management of acute and chronic medical conditions. Provides clinical administrative services supporting Physicians in the clinic location
Qualifications
Minimum of 5 years experience as a PA
PA State Certification.
Advanced training in physical assessment and pharmacology; continuing education in field of practice
