Seeking a Board-Certified Nurse Practitioner to join a mission-based organization that is passionate about changing the way seniors experience primary care just NE of Boston, MA.
- Spanish/English bilingual is required
- All patients are 65+
- Experience working with the senior populations in underserved communities with complex social and medical challenges is preferred
Day-to-day duties:
- Participate in our daily team huddles to discuss patient concerns and care plans
- Perform in-person exams as well as telehealth visits, supported by our EMTs who are in our patients' homes
- Collaborate closely with those who are part of the care team and work to address the patients' social needs
- Communicate and follow up with emergency room providers regarding our patients' history, needs, and care
- Communicate with patients to remain close to their health journey and provide support
- Document visits and clinical notes in our EHR (Athena)
- Provide on-call support for some escalated cases
