- Develop and implement individualized care plans in collaboration with healthcare providers and multidisciplinary teams.
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Provide education and resources to patients and families regarding treatment options, community resources, and support services.
- Coordinate referrals to appropriate services, including outpatient care, acute care, and specialized programs such as infectious disease care or behavioral health services.
- Maintain accurate documentation of patient interactions, assessments, and care plans in compliance with regulatory standards.
- Utilize medical terminology effectively to communicate with healthcare professionals regarding patient status and needs.
- Participate in utilization management activities to ensure appropriate use of resources and adherence to best practices.
Experience
- Previous experience in case management or a related field is preferred.
- Knowledge of medical terminology is essential for effective communication within the healthcare team.
- Familiarity with utilization management processes is a plus.
- Experience in outpatient settings or acute care environments is highly desirable.
- Strong diagnostic evaluation skills are important for assessing patient needs accurately.
