Job Title: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)
Job Specialty: Home Health
Job Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 - 17:00)
Guaranteed Hours: None
Experience: Minimum of 1 full year of Adult Home Health or Hospice experience
License: Must have a valid license
Certifications: CPR, BLS
Must-Have:
- Proficiency in HomeCare HomeBase (HCHB) EMR system within the last 6 months
- Valid driver's license and current auto insurance
- Reliable transportation
Job Description:
- Provide, coordinate, and direct home nursing care according to physicians' orders and agency policies.
- Follow the nursing process to deliver competent and compassionate care.
- Responsible for supervising and evaluating Home Health Aides and other Licensed Practical Nurses/Licensed Vocational Nurses.
- Conduct home visits and attend periodic office meetings.
- Average 5-8 patient visits per day, covering a territory within a 50-75 mile radius.
- Types of cases include Start of Care, Wound Care, Follow-Up Visits, etc.
- Must be able to travel locally and complete all documentation within 24 hours of the patient appointment.
- Participate in rotations for weekend duties and on-call responsibilities.
- Attend weekly meetings at the clinic site.
- Maintain compliance with agency standards and procedures.
