- Effective written and communication skills in English
- Knowledge of population-specific learning principles for patient teaching
- Epic charting experience preferred
Job Description:
- Actively manage patient and family care plans to achieve optimal health goals.
- Utilize resources to deliver quality care safely, efficiently, and cost-effectively.
- Coordinate activities of the healthcare team professionally, fostering a positive work environment.
- Provide care to patients and families that is collaborative, evidence-based, and meets national and facility standards for optimal outcomes.
- Ensure care provided aligns with the organization's standards and enhances the patient and family experience.
- Promote a positive relationship between the healthcare organization and the community.
- Complete required orientation and modules during designated hours.
- No weekend or on-call requirements.
