Job Description
Job Title: RN Case Manager
Job Specialty: Nursing
Job Duration: 13 weeks
Shift: 8:00am - 4:30pm, Monday through Friday
Guaranteed Hours: 40 hours per week
Experience: Minimum of two years of professional nursing experience in a clinical care setting preferred; in-depth knowledge of hospice specialty preferred
License: Unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring; multistate licensure if in a Nurse Licensure Compact state preferred
Certifications: Required department-specific competencies and certifications
Must-Have: Valid driver's license
Job Description:
- Provide care to assigned patients in a caring, safe, and efficient manner.
- Offer leadership to staff and ensure adherence to location standards, procedures, and individualized patient plans of care.
- Evaluate outcomes based on integration among established clinical, financial, and utilization data.
- Demonstrate knowledge of hospice palliative care, hospice philosophy, and scope of services.
- Recognize cultural attitudes and values towards death and dying; identify the five stages of the grieving process.
- Explain advanced directives, clinical requirements, and interdisciplinary services available.
- Utilize independent nursing judgment when integrating health care.
- Participate in creating the plan of care and ensure its implementation by the interdisciplinary group.
- Utilize age-specific structured care methodologies, such as protocols, integrated clinical pathways, guidelines, and standards of care.
- Conduct interviews and assessments of potential residents in compliance with regulations, policy, and procedure.
- Use assessment information for admission and continued stay in the hospice community.
- Establish a service plan for each resident, and evaluate it on an ongoing basis.
- Communicate effectively with patients, residents, family members, and the healthcare team.
- Demonstrate independence, assertiveness, and critical thinking when working with patients and coworkers.
- Document clearly and solve problems efficiently.
- Exhibit self-motivation, work well with little direction, and adapt teaching to age groups.
- Facilitate groups and implement projects when necessary.
- Establish priorities and possess strong flexibility and organizational skills.
- Be knowledgeable about reimbursement for services provided.
- Required to work primarily during the day, scheduled weekends, and occasionally evenings.
- May be exposed to communicable or infectious diseases, hazardous materials, and injury from performance of assigned duties.
- Subject to multiple sensory and environmental stressors.
