Job Description:
- Engage in data collection and case management processes.
- Focus on discharge planning and patient progression.
- Communicate effectively with medical teams to ensure optimal patient care.
- Maintain patient ratios of 1:25.
- Adhere to dress code, which includes business casual or navy scrubs.
- Participate in a rotating schedule for weekends and holidays.
- Work closely with a team that includes 1 RN and 1 Social Worker during day shifts.
- Display an open-minded and proactive personality to thrive in the environment.
