Location: Denver Colorado
Pay rate: $29.69 - $50.46
Shift: Monday through Friday

As a part of Uptown Community Health Center, Bruner Family Medicine sees patients of all ages from all backgrounds of life for Primary Care services. As the main clinic for a Residency program we have over 40 different Providers who rotate through our clinic.
The Care Manager of Social Work is pivotal in coordinating care and services for patients identified as needing assistance or meeting Care Management criteria. An intricate component of this role is collaborating with all members of the multidisciplinary team in the health center. Information is gathered via in-depth assessment that provides the healthcare team with the patient's current level of function and access to community resources and available support systems. The Care Manager uses this information to assist the care team in developing a plan of care, which includes but is not limited to self-management activities, assuring appropriateness of services, assuring individualized support and education, determining the need for continued services, planning for care transitions, and identifying and connecting patients/families with available community resources if needed. The care manager will support the multidisciplinary care team as required for patient management. Abilities that are valued in this role are motivational interviewing and strong communication skills in a team setting.
Role Specific
Licensure / Certificates
Successful completion of Level 2 Patient Navigation skills training (Patient Navigator Training Collaborative) if applicant does not have a Bachelor's Degree.
Qualifications
Required:
High school diploma or GED equivalent
One (1) year of Care Management experience in a primary care setting (or equivalent)
Preferred:
Bachelor's in social work
Licensed Clinical Social Worker OR Licensed Professional Counselor OR Licensed Marriage and Family Therapist
Two (2) years experience in healthcare
Experience with Epic application and health record.
Experience and understanding of Microsoft Office.
Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals.
Bilingual English / Spanish, complete comprehension, literacy, and fluency in both languages

Role Specific
- Identifies patients requiring care management and takes the lead as care manager for those requiring intervention, especially with socially complex cases.
- Demonstrates knowledge of growth and development principles over the lifespan and can differentiate the required care based on the patients' current place on the life growth and development flow.
- Utilizes motivational interviewing skills to support health behavior change in patients.
- Interview patients and families to assess current strategies for self-care and available support systems to manage their health along with support, tracking, and intervening with high utilizers.
