Summary:
Reporting to the Director of Clinical Transformation the Behavioral Health Program Manager, manages the Adult and Pediatric Behavioral Health and Community Health programs and coordinates and develops behavioral health care services that improve the quality of care and health of Brown Health Medical Group Primary Care patients.

Designs, implements and expands comprehensive behavioral health and community health services using quality standards, training, monitoring, and feedback that is consistent with Brown Health Medical Group Primary Care strategic vision.
Develop and analyze Brown Health Medical Group Primary Care's outcomes data and develop plans to improve performance as it relates to contracts and other quality initiatives
Hires, trains, develops and evaluates adult behavioral health and community health team members.
Oversees daily workflows, weekly reports, and team huddles for members of the behavioral health team, including primary care social workers, community resource coordinators, and behavioral health navigators
Compile and analyze program data involving the performance and outcomes of the Behavioral Health Program
Coordinate behavioral health integration into cross-departmental workflows and programs, including the Accountable Entity and Remote Patient Monitoring programs
Develop, establish, and maintain partnerships with external behavioral health and community-based organizations
Ensure continuity of care through different systems of care, with an emphasis on behavioral health care coordination
Collaborate with primary care providers, patients, and members of the healthcare team, including required healthcare services outside of Brown Health Medical Group Primary Care
Works effectively with external stakeholders (e.g. payers, government agencies, community partners) as needed to share data, collaborate or otherwise represent the interests of the company
Performs other job-related duties as assigned

EDUCATION:
Master's degree in mental health-related field
RI Licensed Independent Clinical Social Worker LICSW) required
EXPERIENCE:
Minimum of 5 years experience in healthcare leadership with prior experience working as a behavioral health clinician in a medical setting such as a primary care office or hospital-based setting
Prior experience working as a behavioral health clinician in a medical setting such as a primary care office or hospital-based setting
Excellent interpersonal skills and ability to work with and through others to achieve results
Ability to participate in ongoing quality assurance and improvement processes
Ability to communicate compassion while maintaining equanimity
Ability to work collaboratively with a wide range of internal and external stakeholders
Ability to work both on a team and independently, with the ability to self-direct
Advanced proficiency with computer programs and other technology (i.e., Medical Record, Microsoft Word, Excel and Access, and Web-based applications)
Possess strong work ethic and dedication to the well-being of patients and families
Ability to manage crises and/or support staff in crises up to and including life-threatening situations
Ability to triage effectively every day as well as in high-stress situations while maintaining equanimity
SUPERVISORY RESPONSIBILITY:
Supervises up to 15 FTEs
