This leadership role is responsible for ensuring the delivery of high-quality care and seamless patient transitions throughout the hospital continuum.

Key Responsibilities:
- Oversee care facilitation, case management, utilization management, and discharge planning functions
- Develop and implement the hospital's Case Management program to ensure quality care and positive patient outcomes
- Lead a team of case managers, ensuring effective coordination of patient care across all service areas
- Foster strong relationships with physicians, staff, and external partners
- Manage departmental performance, ensuring compliance with regulations and quality standards
Required Qualifications:
- Active RN license in Montana
- Five years of experience in case management within a healthcare setting
- Proven leadership skills with excellent communication and customer relations abilities
Preferred Qualifications:
- Master's degree in Nursing, Healthcare Administration, or related field
- 3-5 years of supervisory experience in care management or case management services
- Formal training in case management
