Sr Clinical Transformation Specialist

Summary:
Designs and executes on a robust, consistent quality program, consisting of metric analysis, reporting, and workflows to meet requirements for Value-based Care (VBC) contracts and the Medicare Shared Savings Program (MSSP) for the NewHealth Collaborative (NHC) practices that are part of the Accountable Care Organization (ACO)
Provides limited high-level consultation and advice for practices that pursue PCMH on the process
Guides ACO members through the quality program process to meet the principles and objectives similar to a PCMH program for all NHC practices
Leads and educates on standardized clinical quality outcomes for VBC and MSSP through providing:
o Analysis and consultation around improving workflow to meet VBC and MSSP requirements
o Quality improvement project design, execution, and support
o Support to achieve VBC initiatives / requirements, using a variety of EHRs, ie, Epic for SHMG - other EHRs in the independent, community practices
Teaches and assists practices to execute on coding to obtain credit for VBC and MSSP requirements
Works with independent providers and internal Summa Health teams to identify and resolve barriers to access to care at Summa Health
Leads and teaches Clinical Transformation Specialist
Formal Education Required:
a. Bachelor's degree: Nursing, Health Science, or Related Field
b. Master's degree in Nursing, Health Science, or equivalent combination of education and/or experience preferred
Experience & Training Required:
a. Three (3) years of progressively increasing leadership experience in a healthcare clinical related environment
b. Project Management certification/training and/or experience preferred
Essential Functions:
1. Responsible for developing and executing on a robust, consistent quality program, consisting of metric analysis, reporting, and workflows to meet requirements for Value-based Care (VBC) contracts and the Medicare Shared Savings Program (MSSP) for the NewHealth Collaborative (NHC) practices that are part of the Accountable Care Organization (ACO
2. Keep informed of new developments and industry trends related to quality improvement and clinical integration. Provide recommendations needed for decision making and resource planning.
3. Build and maintain strong relationships to engage practice leaders and staff in tactics and strategies to positively impact access, quality, cost and patient experience to achieve identified goals.
4. Understand value-based contract quality requirements and ambulatory practice operations - serve as a resource for practice providers and staff. Regularly identify educational needs and proactively seek to address challenges and barriers.
5. Work with practices to redesign and transform the delivery of care to conform to established standards. Facilitate clinical transformation by leading initiatives within NHC practices. Prioritize and plan improvement activities and projects leading to or maintaining certification or other associated activities.
6. Implement population health tools through the use of practice EHRs, reporting and registry capabilities, patient portals. Adopt evidence-based practices for chronic condition care. Measure the care provided to patients. Measure patient and family experiences of care and support action plans to improve results. Use benchmark and internal performance data as the basis for practice improvement activities. Analyze and make recommendations to modify workflows to achieve quality metric goals.
7. Compile appropriate reports and audits needed for transformation. Report on project status, including barriers, regularly. Manage ad hoc projects as needed.
8. Act as subject matter expert for MSSP quality reporting requirements, planning, presenting results and opportunities, identifying workflow barriers, and working with SHMG and independent NHC provider groups to extract their quality measure data.
9. Provide annual value-based contract quality metric education, workflow suggestions, and performance monitoring to SHMG and independent NHC provider groups directly and through the NHC Quality Collaborative workgroup meetings.
10. Guide the work and progress of Clinical Transformation Specialist. Coordinate with Network Advocates to assist SHN and NHC providers with patient barriers to access care at Summa Health as well as clinical information improvement flows.
11. Obtain reports from EHRs and Support from IT teams as needed.
3. Other Skills, Competencies and Qualifications:
a. Knowledge of regulatory programs, quality metrics and reporting, to meet the requirements for VBC contracting and MSSP requirements
b. Possess an understanding of VBC contracting and payer requirements
c. Knowledge of PCMH principles and how they mesh with VBC and MSSP requirements
d. Possess understanding of the build of electronic health record (EHR) quality reporting
e. Possess understanding of the components of quality reporting, ie, QRDA files, Press-Ganey surveys, administrative / claims submissions
f. Population Specific Competency: Ability to effectively interact with populations of patients/customers with an understanding of their needs for self-respect and dignity
g. Knowledge of improvement science technology, rapid cycle quality improvement
h. Excellent interpersonal and communication skills, both written and verbal
i. Ability to develop and manage a project plan to include resources and deadlines
j. Experience making presentations to large and disparate groups
k. Excellent meeting facilitation skills
l. Coaching and consulting skills
m. Ability to easily adapt to changes in both individual project(s) and the health care environment
n. Problem-solving skills with the capacity to anticipate and identify ever changing needs for the practices as they arise
o. Ability to work with little or no direct supervision & manage work with strict deadlines
p. Ability to mentor and teach staff (internal) and external customers in the practices
