Job Title: Licensed Practical Nurse (LPN) - Full Time
Job Specialty: Long-Term Care (LTC)
Job Duration: 14 Weeks
Shift: Day/Night Rotation, 5x8 per week, Every Other Weekend and Holiday Required
Guaranteed Hours: 40 Hours per Week
Experience: Minimum 1 Year in an LPN Capacity at an LTC Facility
License: Valid LPN Licensure
Certifications: Basic Life Support (BLS) Certification through AHA or RQI
Must-Have:
- Capability to provide care to adult and geriatric patient groups
- Ability to collaborate effectively with interdisciplinary teams
Job Description:
The LPN must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served. Responsibilities include:
- Providing direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse.
- Demonstrating knowledge of the principles of growth and development across the lifespan.
- Assessing data reflective of the patient's status and interpreting information to identify patient requirements relative to age-specific needs.
- Collecting data relevant to the resident's physical and psychosocial condition consistently and communicating findings to the RN and other Interdisciplinary Team members.
- Contributing to the development of a culturally relevant plan of resident care, incorporating nursing diagnosis and interventions to achieve desired outcomes.
- Recommending revisions to the care plan after evaluating resident outcomes and determining the need for referrals.
- Maintaining current knowledge and competency in nursing practice through continuing education and in-service programs.
- Participating in the mentoring and orientation of staff and considering factors related to safety, cost, and impact in planning and delivering care.
- Providing leadership in the professional practice setting under the RN's direction.
- Ensuring a healthy, safe environment while promoting customer satisfaction.
- Demonstrating knowledge and competency, with an understanding that occupational exposure to blood-borne pathogens is reasonably anticipated.
