Responsibilities

• The Primary Care Physician provides high quality and responsive medical treatment to patients in their home, assisted living facilities, hospitals, and other long-term care settings.
• The physician will perform physical exams, take medical histories, perform and interpret diagnostic tests, and recommend a plan of treatment within the accepted standards of practice and which support the philosophy, mission and purpose of Empath Health and Primary Care practice.

Qualifications

A. Education/Regulatory Requirements:

• M.D. or D.O. degree from an accredited medical school and Board Certified in a related specialty unless the Board Certification requirement is waived by the Chief Medical Officer.
• Current/Active Florida license to practice medicine is issued pursuant to Florida Statute 458 or 459v.
• Must be eligible to receive Hospital Staff privileges at one or more hospitals commonly serving patients in the service area unless the requirement is waived by the Chief Medical Officer.
• Current Federal DEA registration.
• Valid CPR Certification required.
• No Medicare, Medicaid, or other Federal Program limitations or restrictions, including no DEA or other prescribing limitations.

B. Skills:

• Minimum of two (2) years' recent active geriatrics practice or a minimum of (5) years of internal medicine or family medicine outpatient experience.

D. Language Skills:

• Bilingual (English/Spanish) preferred.
