Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently looking for a full-time RN Care Manager to join our team, available for both onsite and hybrid work at our Poughkeepsie site.

Position Summary

The RN Care Manager is responsible for coordinating screenings, assessments, care planning, and providing care management services to patients identified as having complex chronic care needs. Acts as a connector between patient needs and the resources provided by the care management program. The RN care manager will promote self-care management and assist with access to care. The RN care manager works with patients through direct face to face patient contact or telephonically. Provides guidance and leadership in the care management program to those in the healthcare organization as well as ensuring the patients receive the best possible care.

Essential Duties and Job Responsibilities:
  • Interview patients to gather their health history, including details about current complaints and past and present medication use, ensuring proper patient identification; document the information in patient charts (must be time-stamped and/or initialed).
  • Maintain patient medical records and document the following: nursing observations, vital signs, reactions to tests (e.g., PPD results, in-house testing), and other relevant information.
  • Prepare equipment and assist physicians during patient treatments and examinations.
  • Initiate follow-up as necessary and instruct patients on healthcare issues as indicated by the physician, diagnosis, or patient inquiries.
  • Assess and triage patients as they present, either by telephone or in person.
  • Demonstrate knowledge of growth and development principles across the lifespan and provide appropriate care.
  • Attend mandated training sessions and stay updated on all nursing procedures, including relevant protocols.
  • Provide organizationally approved patient education materials, including information on health promotion and disease management.
  • Assist in maintaining patients' health and wellness, preventing secondary disease complications by offering education, counseling, and support.
  • Handle protected health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
  • Collaborate with interdisciplinary teams to assess patient needs and deliver care for identified populations upon discharge from various clinical settings. Support care transitions by coordinating activities to prevent or reduce the use of emergency services or hospitalizations.
  • Ensure ongoing and timely reassessment and follow-up of all care plans on a scheduled basis, with timely updates and documentation of patient progress toward established goals.
  • Promote the coordination of relevant health and social care services to eliminate barriers related to social determinants of health.
  • Identify, track, and manage care for designated risk-stratified patients.

Essential Job Qualifications
  • Minimum Education Requirement: Bachelor's degree
  • Minimum Work-Related Experience: 1 - 4 years of experience
  • Minimum Requirement for Licensure/ Certification/Registration:
    • Registered Nurse - NYS
    • CPR Certification
    • Infection Control Certification

Essential Skills & Knowledge
  • Ability to collaborate with other members of the Health Center in order to maintain high quality standards and achieve client satisfaction.
  • Bilingual in both English and Spanish (orally and written) is preferred

Job Type: Full-time
Pay Range: $45.00 - $53.00 per hour
