Summary:

Reporting to the Director of Clinical Transformation, the Manager of Access & Clinical Transformation will work closely with practice management to ensure project management for specialized programs and initiatives, including provider transitions, new patient access, and other operational projects.

Responsibilities:

Ensure consistent, high-quality project management support, including rapid cycle innovation projects.

Manage and oversee operational projects through implementation to ensure goals are met

Facilitate complex planning by revising existing clinical and operational workflows and developing new clinical services.

Support expansion of new patient access across all Brown Health Primary Care practices.

Monitor patient access across all Brown Health Primary Care practices and implement a strategy to increase visit volumes as needed.

Manage all provider transitions within Brown Health Primary Care to ensure patients are provided a new PCP without delaying their care.

Hire, train, and manage the work of the Clinical Support Associates team supporting central scheduling and patient registration.

Collaborate with PCPs, patients, and members of the healthcare team, including required health care services outside of Brown Health Primary Care.

Work collaboratively with corporate colleagues, clinicians, and administrators to provide clinical data on a regular basis to committees and programs.

Organize, attend, and participate in internal and external stakeholder meetings related to clinical and operational projects.

Direct project correspondence by preparing and reviewing project proposals, memos, meeting minutes and emails, including documentation and follow up on important action items from meetings.

Ensure projects adhere to all requirements and documentation is maintained appropriately for each project.

Ensure all deadlines are met, identify and address barriers to meeting project timelines.

Develop and maintain positive relationships with internal and external key stakeholders as needed to collaborate on projects to meet project objectives.

Analyze clinical data to both summarize and identify trends or outliers.

Develop presentations and highlight findings, analysis, recommendations, and action plans.

Perform other duties as assigned by the Director of Clinical Transformation.

Other information:

EDUCATION:

Bachelor's degree or equivalent work experience working in a healthcare setting.

EXPERIENCE:

Minimum of 5 years experience in healthcare leadership with related experience working in a healthcare setting with quality project management.

Related experience working in a healthcare setting with quality project management.

Strong organization, communication, and collaboration skills.

Ability to participate in ongoing quality assurance and improvement processes.

Ability to communicate compassion while maintaining equanimity.

Ability to work collaboratively with various internal and external stakeholders.

Ability to work on a team and independently, with the ability to self-direct

Knowledge of how to use Electronic Medical Record and computerized statistical methods in clinical quality improvement context.

Advanced proficiency with computer programs and other technology (Microsoft Word, Outlook, Excel, and other web-based applications)

SUPERVISORY RESPONSIBILITY:

Supervises up to 5 FTEs
