We are looking for an experienced Care Management Manager to join our team in Fullerton, California. This full-time role will lead a team of Registered Nurses (RNs) and Social Workers (MSWs) to support patients across the care continuum.Job Responsibilities:

• Manage daily operations for the Care Management team.
• Supervise Case Managers and Care Management Coordinators.
• Help patients access care and use resources effectively.
• Work with leadership to improve care coordination and outcomes.

Requirements:

• Bachelor's degree in Nursing or Master's in Social Work.
• At least 3 years of experience in an acute or ambulatory care setting.
• Strong project management and operations skills.

Preferred:

• Master's degree in Nursing or Health Administration.
• Case Management Certification (CCM or ACM).
• 3+ years of management experience in healthcare.
