Overview
The Family Foundations Home Visiting Nurse is a professional caregiver who provides comprehensive nursing services to women and families in the home or community setting. This includes assessment, problem identification, goal setting, and planning applicable nursing interventions for families. The Family Foundations Home Visiting offers short-term home visiting services that are aimed at meeting immediate needs, identifying long-term needs/risk, and connecting the family to available resources including long-term home visiting services. The nurse home visitor will work closely with community, county, and state agencies to provide comprehensive maternal/child homecare services. The nurse home visitors are responsible for maintaining the highest standards in clinical nursing practice and adherence to the policies, procedures, and guidelines of the Carle Health System. Additional responsibilities include the coordination and provision of direct and indirect patient care using the nursing process to meet physical, psychosocial, environmental, and spiritual needs of women and their families.
Qualifications
Certifications: Drivers License ,Proof Auto Insurance, Basic Life Support (BLS) Instructor within 30 days - American Heart Association (AHA); Certified Lactation Counselor (CLC) within 2 years - Academy of Lactation Policy and Practice (ALPP); Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR), Education: College Diploma: Nursing, Work Experience: Related field
Responsibilities
Provides home visits to women and their families in the acute post-partum period.Coordinates the care/services for each familyDevelops and maintains community relationships to support client referrals.Manages the care of a caseload or group of patients effectively and efficientlyAssesses physical, emotional, social, spiritual, and environmental needs of women and their familiesProvides education, support and referral resources as they pertain to maternal health, infant health, household safety/material supports, family and community safety, parenting difficulties, child care plans, parent-child relationship, parent well-being, and parent emotional supportParticipates in review and analysis of reports for achievements and areas for improvement as well as quality improvement efforts.Formulates nursing diagnosis based on nursing assessment and makes appropriate follow up plans and referrals as neededPlans home visits as well as joint home visits with Nurse SupervisorConsults and collaborates with other professionals involved in providing services to women and families.Ability to effectively prioritize and plan work activitiesPossesses the ability to make independent decisions and practice autonomously.Creates a culture of caring, trust, as well as support and growth with families, community members, and team members.Actively engages in professional development. Engages in continuous organizational learning.As new knowledge is acquired through evaluation, study, experience, and innovative approaches, processes and initiatives are refined; Develops and maintains knowledge base of current and applicable federal and state laws, regulations, and accreditation standards; Demonstrates a deep understanding and respect for the community.Performs patient assessments Documents all care in EMR Provides education specific to the post-partum and newborn population

Compensation and Benefits
The compensation range for this position is $29.16per hour - $50.16per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
