Description:

The Certified Diabetes Care and Education Specialist provides individualized diabetes self-management education and training to individuals and groups including, but not limited to, those patients enrolled in the Diabetes in Pregnancy Program according to the scope and standards of practice for Certified Diabetes Care and Education Specialists.

This position will work at our Modesto Women's Clinic. Schedule is Monday - Friday, 8:00am - 5:00pm.

Compensation $43.36 - $50.70 an hour

Essential Duties and Responsibilities
  • Collaboratively develops educational goals, learning objectives and plan for educational content and teaching methods with program participants.
  • Provides educational interventions that utilize primarily interactive, collaborative, skill-based training methods and maximizes the use of interactive training methods.
  • Collaboratively develops an individualized follow-up plan with each program participant.
  • Documents assessment data, educational plan, educational services provided and evaluation results in each participant's educational records.
  • Utilizes a team approach to provide services and collaborates and communicates with team members when needed.
  • Actively participate in quality improvement as it relates to the Diabetes in Pregnancy Program.
  • Utilize electronic population management tools to track and monitor outcomes.
  • Using identified risk criteria contact, assess, and support patients in managing their Diabetes Mellitus.
  • Provide individual case management to patients to include: appointment scheduling with PCP's, identification of resources, group and individual education, patient self management support.
  • Work with the Clinical Team and Outreach in the ongoing development of the Diabetes in Pregnancy Program.
  • Provide staff training and updates to GVHC staff as required.
  • Other duties and projects as assigned.

Physical Demands

The employee must occasionally lift and/or move up to 30 pounds. Must be able to hear patients on the phone and those who are served in-person, and speak clearly in order to communicate information to patients and peers. Must be able to have vision that is adequate to read memos, a computer screen, and administrative documents. Must have high manual dexterity.

Work Environment
  • Exposure to biohazards, including infectious material and waste and any other conditions common in a health care environment.
  • Subject to unpleasant odors.
  • The noise level in the work environment is usually quiet to moderate, but may at times be noisy and crowded.
  • Travel may be required at times.

Education/Experience Requirements

Minimum Qualifications
  • Knowledge of community health, nutrition, motivational interviewing, and adult learning.
  • Excellent customer services and interpersonal skills.
  • Working knowledge of Microsoft Office Suite.
  • Written, presentation and training skills.
  • Ability to analyze data and compile reports.
  • Meet deadlines and prioritize tasks.
  • Work independently.
  • Effectively interact with community agencies, patients, and clinical personnel.
  • Ability to augment schedule to meet patient access needs.
  • Valid California driver's license, vehicle insurance, acceptable driving record and reliable transportation

Education/Experience
  • Certified Diabetes Care and Education Specialist credentialed by the National Certification Board for Diabetes Educators (NCBDE).
  • High school diploma or equivalent.
  • Bachelor's Degree in health related field.
  • Current active unencumbered California Registered Nurse or Nurse Practitioner licensure holding active registration with the Commission on Dietetic Registration.
  • Minimum of one (1) year of health education related job experience.
  • Current American Heart Association Basic Life Support Cardiopulmonary Resuscitation certification.
