What will I do as an RN Unit Coordinator for The Wesley Community?
  • Support the Health Care Center Administration and promote good-will by interpreting Nursing Service and Facility Policies and Procedures to residents, families, visitors and personnel.
  • Maintain good communication with other departments to provide a high quality of life for the residents.
  • Maintain good communication with all Nursing Supervisors and other shifts to assure continuity of resident care.
  • Maintain records and furnish reports as requested.
  • Participate in strategizing for long- and short-term planning through attendance at management meetings and active participation on committees.
  • Participate in surveys/inspections made by authorized government agencies.
  • Make reports/recommendations to the Director of Nursing and Assistant Director of Nursing as necessary, concerning the operation of the unit.
  • Perform as a Team Leader and/or Medication Nurse, responsible for administering injections, suppositories, enemas, and medications associated with wound care
  • Provides adjunct care by administering enemas, douches, non-sterile dressings, surgical preps, ice packs and heat treatments.
  • Maintains patient stability by checking vital signs and weight, testing urine and recording intake and output information.
  • Help maintain the integrity of resident care by assigning, directing, and/or assisting with direct resident care and nursing procedures.
  • Perform direct resident care functions including but not limited to activities of daily living and other basic care needs such as AM/PM Care, as well as perform resident assessment, interviews, wound rounds as well as respiratory therapy.
  • Providing physical support for residents with daily activities and personal hygiene, including bathing, dressing, getting out of bed, and toileting as well as answer resident call lights.
  • Provides for activities of daily living by assisting with serving meals, feeding patients as necessary and ambulating, turning, and positioning patients
  • Evaluate, plan and organize nursing care according to established policies by such activities as daily unit rounds, making rounds with physicians, assuring prompt implementation of doctor's orders, assuring informative and pertinent charting, MDS+ completion, and reporting.
  • Make provision for and assume responsibility for the continuity of good nursing care of residents on all shifts by establishing individual 24-hour nursing/comprehensive care plans.
  • Review, analyze and modify nursing/comprehensive care plans to meet changing resident conditions on a monthly basis and additionally as indicated.
  • Responsible for ongoing Quality Assurance on unit:
    • Meal Site Surveillance
    • Skin Care - prevention, early detection, treatment
    • Restraint use
    • Falls prevention/safety
    • Infection Control
    • Environmental surveillance (report card checklist)
    • Looking at outcomes of care - grooming, personal equipment condition
  • Is responsible for directing any clinical emergency situation which may arise.
  • Assure supplies and equipment are properly used, stocked, and available on the unit; equipment is in good working order and the location of supplies and equipment is familiar to staff.
  • Advise the Day Supervisor of changes in resident status/resident needs and unit needs throughout the day.
  • Meet quarterly with staff on all shifts for communication and problem-solving to promote high-quality resident care.
  • Consult with the resident's physician in planning resident care, treatment, rehabilitation, etc., as necessary.
  • Notify the resident's attending physician and next-of-kin when there is a change in the resident's condition.
  • Work with the facility's consultants and implement those changes authorized by the Director.
  • Authorize the use of restraints when necessary and in accordance with our established policies and procedures.

Registered RN Unit Coordinator Requirements:
  • Graduation from an accredited School of Nursing.
  • Bachelor of Science in Nursing preferred.
  • Current RN license.
  • Recent supervisory or charge nurse experience necessary.
  • Geriatric Nursing experience preferred.
  • Experience in rehabilitation or with chronically ill preferred.
