Job Description: The Case Manager provides social case management support as a primary care provider to the client population. The population targeted for this project are those who have difficulty engaging or have disengaged from care, involved in substance abuse and who are impacted by substance use disorder and Mental Health issues. Utilizing an outreach approach, the case manager provides direct client services including psychosocial assessments on intake, addiction assessments, client goal setting and skill building, and coordination and support of health and social services. A model of recovery services is utilized for those challenged with an addiction. The case manager acts as a client advocate while working with other health care professionals to provide services to this diverse population. The ultimate goal of the Case Manager is to help clients establish a community based medical team and participate in collaborative planning., to achieve the best health possible. This may include stabilizing their mental Health and substance use disorders.

- 1 year experience working in a health related field
Education
- Bachelor of Social Work (BSW)
Licenses
- Valid Class 5 driver's license

- Knowledge in HIV, Hep C and addiction recovery
