Job Title: Physical Therapist

Job Specialty: Home Care

Shift: 8-hour days

Guaranteed Hours: 40 hours per week

Experience: Minimum of 1 year experience as a Physical Therapist preferred; Home healthcare experience preferred

License: Current state license to practice as a Physical Therapist

Certifications: CPR certification required prior to patient contact

Must-Have: Reliable transportation with auto insurance and valid driver's license; excellent communication and interpersonal skills

Job Description:

- Responsible for visiting patients in their home to perform therapeutic evaluations, exercises, and treatments to restore function and prevent disability following illness or injury.
- Evaluate patients' physical and functional status, including musculoskeletal and neurological systems, and mobility skills in daily activities.
- Analyze evaluation results and set appropriate short- and long-term goals to develop patients' care plans.
- Select, administer, or monitor treatment, including modalities, therapeutic exercises, transfers, gait training, functional mobility training, and home safety.
- Determine and recommend appropriate assistive devices and conduct patient and family teaching as needed.
- Reassess patients' response to treatment, adjust programs accordingly, and maintain ongoing communication with healthcare professionals.
- Evaluate the effects of physical therapy treatments and update care plans as needed.
- Maintain accurate and timely documentation in electronic medical records.
- Determine, instruct, and issue appropriate home programs for patients.
- Adhere to universal precautions and infection control policies.
- Attend patient care conferences, in-service education classes, and meetings as required.
- Support continuous quality improvement efforts to improve patient outcomes and satisfaction.
- Develop and maintain positive working relationships with department personnel, patients, students, and outside agencies.
