The MDS Coordinator is responsible for ensuring the accurate and timely completion of the Minimum Data Set (MDS) assessments and care plans for residents in compliance with federal and state regulations. This role serves as a liaison between interdisciplinary team members to ensure appropriate care planning, quality assurance, and reimbursement optimization.

MDS Coordinator (RN) Requirements:
  • Active Registered Nurse (RN) license in VA
  • At least one year of experience in supervisory capacity in a hospital or extended care facility, including experience in geriatric nursing.
  • Prior MDS coordination or RAI process experience strongly preferred.
  • Strong clinical knowledge and excellent decision-making skills.
  • Compassionate, empathetic, and patient-focused.
  • Ability to lead and supervise a team of nursing staff.
  • Excellent communication, organizational, and interpersonal skills.
  • Ability to maintain accurate records and handle sensitive information confidentially.

MDS Coordinator (RN) Responsibilities:

  • Coordinate Interdisciplinary Care Planning Meetings and the entire MDS process.
  • Maintain the Care Planning Schedule and assure that all chart documentation and Care Plans follow State and Federal Regulations regarding the MDS process.
  • Assure accurate and timely completion of MDS documentation by all departments and the weekly electronic submission of MDS documentation as required in a timely manner to assure compliance with State and Federal Regulations.
  • Participate in Monthly and Quarterly QAA Committee and Committee functions. Serve on Subcommittees, collect, and analyze Date whenever requested by QAA Coordinator, DON, and administration.
  • Serve as Coordinator of QAA Wound and Skin Subcommittee. Responsible for accurate and timely submission of associated weekly reports as set forth in the Wound and Skin Policy Manual. Assure appropriate documentation is obtained for "unavoidable" pressure sores.
  • Coordinate weekly Medicare Utilization Review meetings and maintain notebook of Skilled Residents' length of stay and status changes (different from the QAA lists of Medicare residents maintained by DON).
  • Perform Quality Indicator Analysis, PPS Analysis, and any other computerized analysis reports periodically or whenever requested by DON or administration.
  • Works as a Supervisor or Staff RN whenever requested by DON or administration.
  • Assist GNAT Instructor with GNAT clinical instruction whenever requested by DON or administration.
  • Promote harmonious relationships among co-workers and other departments, including the administration, as well as with residents, families, and visitors.
  • Demonstrate by example and coordinate efforts between DON and Nursing.
  • Supervisor/Nursing Staff to serve as a communication channel from administration to the nursing department to assure personnel conformity to Fifth Star Care Management policy.
  • Demonstrate by example and coordinate efforts between DON and Nursing Supervisor/Nursing Staff to serve as a communication channel to administration in conveying staff's suggestions for effective change.
  • Interpret principles of good management to nursing personnel and serve as a role model in the implementation and application of these principles in daily work.
  • Provide opportunities for staff to solve problems and to develop management and nursing skills under appropriate supervision.
  • Recognize individual and group strengths and utilize strengths to improve weaknesses.
  • Attend all facility meetings as requested or directed by the administration or DON.
  • Any other duties as requested by DON and Administration.
