Description:

POSITION PURPOSE
  • Director of Community Health & Well-Being (CHWB) leading the CHWB team at Chelsea Hospital.
  • The Director of Community Health & Well-Being (CHWB) works in collaboration with CHWB Vice President for the Region to ensure that the region is fulfilling its commitment to being a national leader in optimizing wellness and equity and eliminating disparities in our communities.
  • This position will set strategy, leverage resources and partnerships focusing on those who are poor and vulnerable, and addressing the social determinants of health. This position provides oversight of community health programs and personnel, and acts as a liaison to community agencies in determining community needs.
  • In addition, this position is accountable for regional CHWB priority strategic aims, community benefit compliance, community benefit reporting, and progress toward achievement of the CHWB standard indicators.


MINIUMUM QUALIFICATIONS
  • Bachelor's degree in Health Care Administration, Public Health, Community Health or related field is required. Master's degree is preferred.
  • Seven (7) years of community health and well-being experience with demonstrated progressive responsibilities in strategic planning, fiscal management and operations. Five (5) years of experience in a team or initiative leadership capacity is required.
  • Exceptional leadership, management and facilitation skills. Demonstrated ability to skillfully counsel, collaborate and negotiate; inspire and build confidence in others; create alliances; obtain support and respect from diverse groups and foster an understanding and commitment to the organization. Ability to adapt style and approach to match the needs of varying teams, individuals and situations.
  • Strong working knowledge of health disparities, social determinants of health, uncompensated and undercompensated care, Community Benefit, community and coalition building, public policy development and advocacy, Triple Aim and population health management concepts.
  • Excellent oral and written communication, persuasion, and negotiation skills necessary in order to facilitate communication between diverse groups; to effectively identify, assess and facilitate improvements and resolutions, and to present advisory, persuasive and authoritative recommendations to all levels of the organization.
  • Considerable experience and expertise in planning, formulation, execution and evaluation of CHWB strategies and programs.


WHAT YOU WILL DO
  • Knows, understands, incorporates and demonstrates the Trinity Health Mission, Vision and Values in behaviors, practices and decisions.
  • Directs personnel, operational and financial activities over multiple hospitals, inclusive of Faith Community Nursing. Responsible for making decisions and managing risk that impact the functional area or program operations.
  • Oversees staffing and succession planning activities, develops, coaches, mentors and motivates colleagues. Fosters and sustains a challenging, inclusive and rewarding work environment where talent is developed and diversified. Facilitates diverse member interests, teams and groups through strong leadership, communication, collaboration and negotiation abilities to mutually agreeable resolutions. Ensures compliance with established Hospital and personnel policies when operating the Department.
  • Develops long and short term goals, objectives, plans and budgets for all programs within CHWB to ensure continued quality of services through planning across the region with other functional areas.
  • Leads the Ministry's CHWB council that works collaboratively to implement and support Trinity Health national goals, address social determinants of health and promote health equity.
  • Serves as a CHWB knowledge resource and convener of stakeholders, representing Ministry's commitments to community health improvement and partnerships.
  • Oversees and develops measurable community health improvement plans and implementation strategies, in collaboration with Ministry senior leadership, that align with the community health needs assessment and Trinity Health strategic plan.
  • Identifies and establishes rapport with funding sources such as foundations, state and federal agencies and establishes reporting systems for each funded initiative in order to ensure that programmatic designs comply with all applicable rules and regulations.
  • Represents Ministry at professional or community organization meetings and on committees at local, state and national level in order to assist in developing new programs and policies for improved delivery of health services those who are poor and vulnerable.
  • Oversees the development and implementation of a Community Health Needs Assessment (CHNA) every three years in partnership with community stakeholders, placing particular emphasis on the needs of those who are poor and vulnerable and addressing social determinants of health.
  • Develops and submits an annual Community Health & Well-Being budget that aligns annual spending plans with the Ministry's goals and objectives while also ensuring CHNA implementation efforts are resourced.
