Primary Function

The Clinical Pharmacy Specialist, Geriatrics works with the geriatrics consult service (including geriatricians, hospitalists, nurse practitioners, medical residents, and nursing), and outpatient clinicians to enhance pharmacotherapy and associated outcomes for patients of advanced age. The geriatrics specialist will also be involved in geriatric-friendly institutional initiatives through committee participation and partnership with quality department colleagues. The geriatrics specialist will also provide pharmacy oversight in the institutions hospital-at-home program, providing the team with pharmacotherapy recommendations, assisting in medication processing and workflow, and developing/maintaining standard operating procedures for internal use within the pharmacy department, as well as any associated pharmacy policies.

Job Qualifications and Specifications

Doctor of Pharmacy (PharmD) degree with 5 years of clinical pharmacy practice experience or post-graduate residency training is required. Current RI Pharmacist license is required. Board Certification in Geriatrics Pharmacotherapy (or Board-eligible) is preferred but not required.
