The Staff Geriatric Physician provides primary care, including routine health maintenance, management of chronic conditions, and management of acute illness for PACE (Program for All Inclusive Care for the Elderly) participants in collaboration with the interdisciplinary team. Position is based at our Elder Service Plan, 1135 Morton Street, Mattapan location.

Role:
The Staff Geriatric Physician provides primary care, including routine health maintenance, management of chronic conditions, and management of acute illness for PACE (Program for All Inclusive Care for the Elderly) participants in collaboration with the interdisciplinary team. Position is based at our Elder Service Plan, 1135 Morton Street, Mattapan location.
Key elements of PACE model of care include:
- Our Interdisciplinary Team manages the care of our frail elders and supports each other and the Elder Service Plan participants in achieving care goals in small patient/participant panels (75-100 patients).
- Participants attend our center for Adult Day Health activities as well as receive on-site clinical care including primary care, behavioral health and rehabilitation.
- Our physicians follow our elders throughout all locations of care including sub-acute, long-term and home care settings; regular home visits are encouraged and time is allotted for this effort.
Responsibilities:
- Serves as a primary care provider for a panel of participants.
- Organizes and manages the care of participants, including outpatient visits, communication with participants by mail, email or phone, inpatient care, referral for specialty care and provision of or referral for participant education for self-management.
- Provides timely and expert care to participants with acute illness.
- Manages common chronic illnesses, including acute complications and routine follow-up.
- Participates in an on-call system with other providers to ensure 24-hour access to care for our participants; effectively triages problems described on telephone calls, determines the need for admission of participants to the hospital when presenting to the emergency room, through either telephone consultation with ER staff, or in person and provides communication to providers regarding their inpatients and on-call activity.
- As appropriate, provides high quality in-hospital care for participants; provides in-patient care for other providers, when on-call.
- Provides care at the PACE center, in patient homes, and in skilled nursing facilities.
- Participates in provider staff meetings, departmental meetings, general staff meetings, trainings, interdisciplinary team meetings and quality improvement activities.
- Supervises mid-level practitioners; reviews problem cases, is available for questions, reviews medication prescribing, and provides teaching.
- Makes appropriate, cost efficient referrals and admission decisions and refers primarily to contracted specialists; provides guidance and monitoring of systems necessary for the function of a capitated care program.
- Updates Electronic Medical Record (EMR) appropriately using proper coding and documents care accordingly; accurately and timely documents care in the medical record with progress notes, lab follow-ups, communications with participants; maintains a current problem list, medication flow sheet, immunization record and other appropriate information.
Requirements:
- Completed accredited Physician Residency in Internal Medicine or Family Practice with additional Fellowship training in Geriatric Medicine required
- Current MA License, DEA, MCSR, Board Certified/Board Eligible, current Basic Life Support (BLS) certification
- Proficient in MS Office (Word, Excel, Outlook) and EMR (EPIC desired)
- Experience working with a multidisciplinary team.
- Experience in the development and implementation of geriatric programs.
- Strong clinical, leadership, interpersonal and organizational skills.
- Knowledge of community resources preferred.
