Post Acute Program Manager Job Summary:
The post-acute program manager will be responsible for assisting with the growth, evaluation, and sustainability of BridgePoint Healthcare. The position is responsible for developing and executing programs that engage the community in support of the BridgePoint Healthcare mission. The post-acute program manager of Community Outreach is responsible for the planning and execution of a wide variety of initiatives to promote LTAC and SNF programs. This role will work with a variety of stakeholders. The main focus of this position is to increase census goals of LTAC and SNF programs for BridgePoint Healthcare through increasing market awareness in the community and building relationships with community organizations, community physician groups and other clinical personnel to achieve the goals set by BridgePoint Healthcare. The person must be able to effectively communicate the organization's message and goals to various groups. The post-acute program manager is responsible in building relationships with local organizations and/or businesses that might be willing to partner with BridgePoint Healthcare on its projects and initiatives. The post-acute program manager is flexible, responsible for assignments, supportive of team efforts and willing to travel throughout the region.

Qualifications

- BS required
Licenses/Certification:
- Valid Driver's License
Experience:
- Using independent judgement and discretion, the post-acute program manager must have experience as a trained healthcare professional (RN, LPN, RT, OT, PT, or Allied Health may be considered depending on prior LTAC or comparable experience). Ability to effectively communicate with all levels of hospital staff, physicians, and patient referral sources is a must. Experience in critical care, trauma, or other specialty setting is preferred. Marketing experience preferred.
