Description:
We are seeking a dedicated Discharge Planner Case Manager for a 13-week assignment.
The role involves providing crucial discharge planning and case management services within a Mother/Baby unit.
The work schedule consists of 8-hour day shifts, starting at 08:00 and ending at 16:30, with weekends included in rotation every 5th or 6th weekend.
There is no on-call requirement, but holiday coverage may be needed.
Candidates must have two years of experience and acute hospital discharge planning experience.
Applicants should possess skills in case management, emergency department, labor and delivery/mother-baby, medical-surgical unit, and NICU.
Candidates must be eligible for LMSW/LGSW/LCSW or hold DC RN Licensure, with certain units accepting pending applications with proof.
A BLS certification is required, and all employees must have the flu vaccine.
The position requires a professional appearance with business casual attire.
Orientation includes 28 hours.
