Physical Therapists at the VNH screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.

Physical Therapists at the VNH screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with our interdisciplinary team members and the patient.
The PT will:
- Evaluate the patient's need for physical therapy and develop an appropriate treatment plan.
- Carry out appropriate treatment programs in accordance with agency policies and professional standards of care.
- Communicate with all members of the health care team.
- Document evaluations, treatment goals, and objectives.
- Teach patients and family members the rationale for treatments and procedures.
Minimum Qualifications:
- BS in Physical Therapy
- Must be a licensed Physical Therapist in VT and NH, or hold a compact license.
- One year of experience as a PT in a home healthcare setting
Salary Range (depending on experience):
- Minimum: $77,792
- Maximum: $120,556
