Job Title: Case Manager Registered Nurse
Profession: Registered Nurse
Specialty: Case Management
Duration: 13 weeks
Shift: Day
Hours per Shift: 8 hours (08:00 - 16:30)
Experience: Minimum 2 years required
License: Compact RN License required (pending acceptable)
Certifications: BLS required from the American Heart Association
Must-Have: Acute care experience
Description:
The Case Manager RN is responsible for managing a caseload of 35-40 patients per day.
The role requires real-time documentation of assessments and discharge planning notes.
The expectation is for documentation to occur during the assessment process.
The RN will be part of the discharge planning and care coordination process.
Experience with MCG guidelines is a plus.
Dress code is business casual with a white lab coat or jacket required when on the floor.
The position requires working every other weekend.
The clinician can be floated within the scope to meet facility needs.
Orientation is held every other week on Thursday and consists of 16 hours.
Risk assessments and understanding of social determinants of health are beneficial.
The hospital is a level 1 trauma center and a major teaching hospital.
Flu vaccine is required during the flu season (October 1st - March 31st) unless medically exempt.
The COVID-19 vaccine is not required.
Submission must include candidate profile, work history, two recent manager or charge references, RN license verification, and certifications.
